The shoulder is held in place by a set of five very important muscles that attach to the shoulder blade called the infamous rotator cuff! The mistake so many patients make is to think of rotator cuff injuries as one size fits all when there is actually a long spectrum ranging from just overuse of one of these rotator cuff muscles which puts strain on the tendon where it attaches to the shoulder all the way to a full thickness, complete tear of the rotator cuff muscles or tendon and lots in between.

A Physiotherapist can do various tests and an assessment to establish which part of your shoulder is injured, to what extent your rotator cuff has been affected and if a diagnostic ultrasound scan is needed and even advise where the best place is to have this scan and write a referral.

Most rotator cuff injuries start with the muscle itself and then progress into the tendon and result from incorrect shoulder blade positioning. Therefore Physiotherapy treatment will focus on the muscles where it all began and rehabilitation which involves showing you how to place your shoulder blade in the correct position especially during activities such as working at the computer, driving, DIY and various sports where the shoulder is under a lot of load such as golf, padel, tennis and swimming.

It is important for you to know that when it comes to a tear of the rotator cuff tendon/s it is only 100% necessary to have an operation if it is a complete tear as there are no fibers in close proximity for the body to be able to heal itself and an operation is necessary to reattach or reunite the torn tendon.

If you have a partial tear where some of the fibers are torn but some are intact then, in my experience seeing many patients who have chosen not to have an operation, it is possible for the tendon to heal and function in the shoulder to be restored.

With a partial tear whether you choose to have an operation or not, the recovery time is a long journey and it is very important that you rest the shoulder from activities that put a lot of load on it but equally important that you keep the shoulder mobile and prevent stiffness and the stabilising muscles from getting very weak and this is where Physiotherapy and rehabilitation is very helpful.
